Requirements

A heart to serve patients and families and a passion for providing the best possible care
Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
2 years experience within the last 5 years in chronic disease management/long-term care
Reliable transportation
Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively
