About The Position

The Ponca Tribe of Nebraska is a federally recognized Tribe with its ancestral homeland located in Niobrara, NE. The Tribe does not have a reservation in the classic definition; rather, the Ponca Restoration Act defined its jurisdiction as a ?Service Delivery Area? comprised of 15 counties throughout parts of Nebraska, Iowa, and South Dakota. Within its Service Delivery Area, the Tribe offers a broad range of health, social, educational, and cultural services.This position is responsible for providing primary care services across the lifespan, including assessing, diagnosing medical conditions, prescribing, treating, and educating patients. Works with physicians and other clinicians in a team based approach to patient-centered care. May specialize in a specific type of clinical care i.e. pediatrics, geriatrics, etc. Percentage of clinical hours: 100%.

Requirements

  • Active and in good standing BSN License
  • Masters of Science in Nursing from accredited program
  • Two years of clinical experience as a BSN-Registered Nurse
  • Active Temporary Nurse Practitioner License in good standing
  • Active and good standing APRN License and successful completion of National Certification Exam
  • Computer proficiency in electronic health records systems, Microsoft Word, Excel, PowerPoint and Access programs.
  • Must possess excellent written and oral communication skills.
  • Must have valid driver?s license and be insurable.
  • Must successfully pass drug and alcohol testing, motor vehicle, education, character, and criminal background checks.
  • Able and willing to travel.

Nice To Haves

  • One or more years? experience working with Native Americans or Native American organizations.
  • Knowledge and understanding of Public Law 93-638: Indian Self-Determination Education Assistance Act of 1975.

Responsibilities

  • Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing/presenting illness, risk factors, family history, psychosocial situation, and cultural factors; and performing appropriate physical examination.
  • Orders/ performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed including consultation as needed.
  • Discriminates between normal and abnormal finds to recognize early stages of serious physical, emotional or mentalproblems.
  • Works independently with Full Practice Authority and in collaboration with the care team (clinicians, nursing, and support staff) to make decisions concerning needs of patients. Seeks and obtains input from physicians (internal and/or external) regarding questions and issues pertaining to the treatment and/or care of patients as needed.
  • Provide surveillance over medical practices applied to stabilize chronically ill persons; adjusts treatments within established guidelines recognizing when to refer the patient to the physician or other health team members.
  • Provide emergency services or crisis intervention including life saving emergency procedures in order to stabilize a patient sufficiently to transport to a hospital facility.
  • Evaluate the medical aspects of treatment plans periodically recognizing the need for reassessment by a physician or other health professional.
  • Provide medical care and preventative services, including guidance in nutrition, common illnesses, accidents, child growth and development and child rearing.
  • Consistently completes documentation within the electronic health record to include documentation of patient encounters, entry of relevant patient data, review and signature of labs, care plans, etc. to include communication with patients, specialists and members of the patient?s care team.
  • Exhibits knowledge of evaluation and management (E&M) coding guidelines, and demonstrates appropriate application of those guidelines as evidenced by accurate E&M coding.
  • Exhibits awareness of managed care and health plan requirements in respect to prescription formularies, prescription formularies, and pre-certification requirements for lab or diagnostic tests, participating facilities, and medical necessity criteria. Utilizes appropriate health and medical resources to coordinate treatment plans for patient per requirements of the managed care plans where applicable.
  • Collaborates with other professionals and agencies involved in providing health care and services such as immunizations and disease control, environmental health, health education, health researchers, nursing students and residency training programs.
  • Participate in quality assurance functions of health center including participation and acceptance of leadership roles in a variety of required committees as directed. Serves as a role model and in selected situations precepts clinical staff.
  • Taking a well ordered approach to solving problems and using sound judgment in making decisions despite obstacles or resistance.
  • Assumes responsibility and accountability for time management, professional relationships, and interpersonal interactions within the job setting.
  • Knowledge of principles, practices, methods and techniques in the management of the FLHWC health / Ponca Tribe of Nebraska care delivery system to assist with planning, organizing, implementing, evaluating and recommending changes in programs, functions, services and activities of the FLHWC.
  • Adheres to the policies and procedures of the Ponca Tribe of Nebraska.
