About The Position

The Nurse Practitioner will be responsible for managing a panel of Longevity Health Plan enrolled nursing home level of care members in collaboration with the primary care team. This role involves conducting comprehensive assessments, developing individualized care plans, and ensuring effective communication with the care team and family members. The NP will focus on treating members in place, utilizing clinical practice guidelines, and documenting health statuses accurately.

Requirements

• State unrestricted licensed Advanced Practice Nurse.
• National Board certification as an Adult, Family, Geriatric or Primary Care nurse practitioner.
• DEA License required.
• Active national provider number (NPI) and Medicare provider number.
• Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program or equivalent.
• Minimum of 6 months experience as a Nurse Practitioner.
• Experience working in the nursing home environment is strongly desired.
• Computer/Technology proficiency required.

Nice To Haves

• Experience in hospice palliative care.
• Excellent people skills for engaging with physicians, patients, and families.

Responsibilities

• Provide collaborative primary care and services to a panel of Longevity Health Plan Services.
• Execute and deliver Longevity Health Plan Model of Care activities.
• Develop and implement advanced care planning.
• Facilitate Health Plan clinical operations functions such as prior authorization and medication reviews.
• Serve as a trusted advisor to members, families, and nursing home staff.
• Deliver effective communication and coordination with members' PCPs and care team members.
• Support overall market goals and balance member caseload demands.
• Promote Longevity Health Plan with members and stakeholders.
• Establish meaningful relationships in the nursing home environment.
