About The Position
ComplexCare Solutions (CCS) specializes in providing high-value in-home health assessments for health plans across the nation. Our mission is to enhance member engagement by evaluating their current health status, identifying gaps in care, assessing potential health risks, and exploring care management opportunities. We focus on driving better clinical outcomes through personalized care. This position is ideal for Nurse Practitioners (NPs) and Physician Assistants (PAs) who are looking for a unique opportunity to support their communities, educate members about their health, and help keep them in their homes rather than in hospitals. As a CCS provider, you will have the flexibility to create your own schedule, working between Monday and Sunday from 8 AM to 8 PM. You will conduct in-home assessments, which include a comprehensive review of medications, a physical examination, and discussions about medical concerns affecting the member. Each visit is allotted up to an hour, allowing for meaningful interactions that empower members to understand their health better and prepare for future discussions with their primary care providers. In addition to competitive pay, which ranges from $100 to $135 per hour depending on the type of assessment, CCS offers collaborative physician support, malpractice insurance, completion bonuses, and reimbursement for all mileage based on federal rates. Text reminders will also be sent to members to confirm visits when applicable. This W-2 position is designed for healthcare heroes who are passionate about making a difference in the lives of their patients.
