About The Position
Theoria Medical is leading the charge in healthcare innovation, offering a unique blend of medical excellence and technological advancement, primarily serving the post-acute and primary care sectors. Our extensive network includes multispecialty physician services and RPM, covering skilled nursing facilities across the country. We are looking for nurse practitioners and physicians eager to drive change and deliver superior care. This position is full-time and exempt, with a salary of up to $165,000 annually, and is eligible for a sign-on bonus. The role involves providing high-quality, patient-centered, comprehensive primary care services to patients residing in skilled nursing facilities (SNFs). The provider will be responsible for rounding daily, prescribing medications, and managing clinical care, while ensuring timely documentation of all patient encounters.
