About The Position

Our extensive network includes multispecialty physician services and RPM, covering skilled nursing facilities across the country. We are looking for nurse practitioners and physicians eager to drive change and deliver superior care. This position is full-time and exempt, with a salary of up to $165,000 annually, and is eligible for a sign-on bonus to welcome you to the team.

Requirements

• Must be a licensed medical provider (nurse practitioner or physician).
• Ability to perform essential functions of the position satisfactorily, with or without reasonable accommodation.

Nice To Haves

• Experience with PointClickCare, MatrixCare, and American HealthTech EHR systems.
• Previous experience in a skilled nursing facility setting.

Responsibilities

• Rounding and managing the clinical care of the population at the assigned skilled nursing facility (SNF).
• Rounding daily and prescribing medications and treatments for residents residing at SNFs.
• Providing high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
• Timely documentation of all patient care/encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Completing provider documentation in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
• Being on-site on time for scheduled shifts.
• Maintaining reliable transportation.
