About The Position
Theoria Medical is seeking a dedicated provider to manage clinical care at skilled nursing facilities (SNFs). This full-time, exempt position offers a unique opportunity to deliver high-quality, patient-centered primary care services. The role involves daily rounding, prescribing medications, and ensuring timely documentation of patient encounters. Providers will utilize Theoria’s proprietary electronic health record system, ChartEasy™, and will be expected to maintain reliable attendance and transportation for in-person shifts.
