About The Position

The Nurse Practitioner will work under the general direction of the Medical Director or Physician to provide health maintenance, episodic illness, and chronic disease patient care to patients. This role involves delivering primary medical management through collaboration with a physician, conducting health assessments, and evaluating the needs of patients in the primary care clinic. The Nurse Practitioner will also perform physical exams, order and/or perform laboratory and diagnostic tests, and make clinical judgments regarding results. Additionally, the position includes participating in the development of comprehensive care plans, providing health information and teaching, and acting as a primary care resource to the Multidisciplinary Team.

Requirements

  • Current certification and licensure as Nurse Practitioner in the Commonwealth of Massachusetts.
  • Ability to maintain patient confidentiality at all times as required by the health center and regulations.
  • Experience working with two or more of neonatal, pediatric/adolescent, adult, and geriatric populations.
  • Experience in culturally diverse settings and passionate about community health.
  • Experience working within a multidisciplinary team.
  • Ability to communicate clearly with patients and with staff.
  • Strong leadership, interpersonal, teaching, and organizational skills.
  • Willingness to work evenings and weekends as required.

Nice To Haves

  • Proficient in Spanish, Portuguese, and/or Cape Verdean desirable.

Responsibilities

  • Provide primary medical management for patients through collaboration with a physician.
  • Deliver patient care and patient education appropriate to patient's age, physical ability, educational level, and culture.
  • Conduct health assessments and evaluate the needs of patients in the primary care clinic.
  • Conduct physical exams, order and/or perform laboratory and diagnostic tests, and make clinical judgments regarding results.
  • Perform minor procedures independently.
  • Participate in the development of a comprehensive plan of care for each patient.
  • Initiate appropriate consultation and referrals.
  • Provide health information and teaching with emphasis on prevention and curative treatment.
  • Act as primary care resource to the Multidisciplinary Team and provide in-services for staff as required.
  • Participate in teaching activities with nurses, nurse practitioners, and medical students.
  • Document History & Physical, assessment, and treatment intervention in medical record.
  • Participate in committees for quality of care, standards of care, policies and procedures, and other program development activities.
  • Maintain current standards of practice and seek professional development.
  • Participate in regularly scheduled clinical care, team, and department meetings.
  • Perform other duties assigned by supervisor as required.
