About The Position
Carease Health is looking for a dedicated Nurse Practitioner (NP) with experience in Geriatrics or Internal Medicine to provide care management services to a select group of patients. This part-time role involves brief in-person visits each month, complemented by remote patient documentation supported by a scribe team, allowing the NP to focus on patient care and clinical decision-making.
