About the position
Theoria Medical is seeking a dedicated provider to manage clinical care at skilled nursing facilities (SNFs). This full-time, exempt position offers a competitive salary of up to $165,000 annually, along with a sign-on bonus. The role emphasizes patient-centered care, requiring the provider to round daily, prescribe medications, and maintain timely documentation using proprietary and commonly used electronic health record systems. The position is in-person, with shifts scheduled from 7 AM to 1 PM, Monday through Friday, allowing for flexibility in scheduling. Responsibilities
- Rounding and managing clinical care of the population at assigned SNFs.
- Prescribing medications and treatments for residents at SNFs.
- Providing high quality, patient-centered, comprehensive primary care services.
- Timely documentation of all patient care encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Completing documentation in Theoria’s proprietary EHR system, ChartEasy™.
- Ensuring timely attendance and reliable transportation for in-person shifts.
Requirements
- Valid medical licensure.
- Experience in primary care or related field.
- Ability to perform essential functions satisfactorily, with or without reasonable accommodation.
Nice-to-haves
- Experience with PointClickCare, MatrixCare, or American HealthTech EHR systems.
- Strong communication skills for patient and staff interactions.
