About The Position

A full-time Nurse Practitioner position is available with Liberty Healthcare at the Stockley Center in Georgetown, DE. The Stockley Center is state-of-the-art intermediate care facility which provides a wide array of services to adults who are living with physical and developmental disabilities.

Requirements

• Licensed in Delaware as a Nurse Practitioner
• Proficiency in diagnosing, treating and managing acute and chronic health conditions for adults

Responsibilities

• Daily patient evaluation through rounding
• Conducting examinations and performing diagnostic tests
• Providing preventive care and treating acute and chronic health conditions
• Developing treatment plans
• Prescribing medications
• Collaborating with the facility's Medical Director, nursing team, and ancillary staff
• Actively participating in multidisciplinary treatment team meetings
