Provides specialized assessment, diagnosis, and management of patients with neurodegenerative diseases and cognitive impairment in collaboration with a physician and interdisciplinary team. Functions within the full scope of licensure and is accountable for coordinating and delivering individualized care across all stages of disease progression, including Alzheimer's disease, Parkinson's disease, Lewy body dementia, and other cognitive disorders. Delivers holistic care that addresses the complex physical, psychological, behavioral, and social needs of patients and their families. Develops and implements evidence-based care plans that integrate neurologic evaluation, cognitive screening, medication management, caregiver support, and transitions of care. Maintains strong collaborative relationships with patients, families, physicians, and allied health professionals to promote informed, compassionate, and coordinated care. Adheres to hospital policies, clinical protocols, and applicable state regulations, including the California Nurse Practice Act. Committed to continuous learning and professional development in the field of neurocognitive and geriatric care, and actively contributes to best practices in memory and neurodegenerative care settings.
