About The Position

The Nurse Practitioner is a Registered Nurse with advanced educational preparation for managing the health care needs of patients in a hospice setting. Scope of practice includes patient education, assessment of disease and treatment of illness under protocols developed through collaborative efforts of the physician and nurse practitioner.

Requirements

  • Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. Completion of certificate Family Nurse Practitioner or Masters level Nurse Practitioner program.
  • Licensure: Current RN License in the State of Georgia. Certification as a Family or Acute Care Nurse Practitioner by the American Nurses Association. Recognition by the Georgia Board of Nursing of the right to practice as a nurse practitioner.
  • Experience: Five years of nursing experience as a Registered Nurse with at least two years of clinical experience in an physician office, hospice agency, or acute care facility as a Nurse Practitioner.
  • Skills: Knowledge of all phases of the nursing process. Demonstration of clinical competency in the Nurse Practitioner role. Demonstration of qualities of leadership and sound judgement. Committee to patient/family education in disease prevention and health maintenance. Ability to effectively interpret , implement and support approved hospIce policies, regulations, philosophy and objectives. Ability to document and communicate findings of patient assessments to Hospice interdisciplinary Team.

Responsibilities

  • Performs patient visits requested by Hamilton Hospice:
  • General Inpatient care (GIP, short-term symptom management for patient with terminal illness) with patient admission/stay at Hamilton Medical Center
  • Respite care (Respite care is provided to a hospice patient so the usual caregiver can rest)
  • Face-to-Face (when a patient must have a face-to-face encounter to determine the continued eligibility of that patient).
  • Works in nursing home, patient residence or acute care setting with terminal patients who may be experiencing a wide range of medical problems.
  • Required to work flexible hours.
  • Ability to work with patients and significant others in stressful, emotional situations.
  • Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination.
  • Requires exposure to communicable diseases or body fluids.
