The Palliative Care Nurse Practitioner plays a vital role in delivering compassionate, expert care to patients facing serious, progressive illnesses. Working under the clinical supervision of a licensed physician, this advanced practice provider focuses on improving quality of life through comprehensive symptom management and goal-directed care. As a trusted member of the interdisciplinary team, the Nurse Practitioner provides holistic support—addressing physical, emotional, social, and spiritual needs—while guiding patients and families through complex healthcare decisions.   • Conduct comprehensive assessments, diagnose conditions, and develop individualized care plans in collaboration with the Medical Director.
  • Provide symptom management and palliative interventions across multiple settings (home, hospital, clinic, long-term care).
  • Complete hospice certification visits and participate in limited on-call rotation.
  • Educate and support patients and families, ensuring clear communication of goals of care.
  • Document assessments, interventions, and billing accurately and timely.
  • Maintain compliance with quality standards, continuing education, and organizational policies.
  • Master’s degree in Nursing and completion of an accredited Nurse Practitioner program.
  • Board Certified as Adult, Family, or Gerontology NP
  • Minimum 3 years nursing experience with demonstrated expertise in pain and symptom management for palliative populations
  • Strong ability to collaborate within an interdisciplinary team, including volunteers.
  • Excellent communication and presentation skills for engaging physicians and the public.
  • Proficient in Microsoft Word and Outlook.
  • Hospice and Palliative Care certification preferred.
  • prior experience with terminally ill patients preferred.
