About The Position
HarmonyCares is one of the nation's largest home-based primary care practices, dedicated to providing high-quality, coordinated health care in the home. The Nurse Practitioner will deliver annual risk assessments in a residential setting or via telehealth, within the scope of practice as delegated by the Collaborative Physician. The role involves conducting comprehensive in-home health risk assessments, taking patient histories, determining diagnoses, and providing written documentation of patient visits per NCQA standards.
