Summary: Under the clinical supervision of the Primary Care Physician, the Primary Care Nurse Practitioner (PCNP) will be an integral member of Care Dimensions Primary Care at Home Program. The PCNP has responsibility for providing patients with high quality treatment and services. The PCNP is a licensed independent practitioner who is responsible for meeting, assessing, diagnosing and treating patients in their home and providing them with healthcare services that meet state and national standards.
