About The Position

The Department of Veterans Affairs NY Harbor Healthcare System (NYHHS) is seeking a full-time Nurse Practitioner for its Pain Clinic, within Primary Care. This position supports all three campuses (Manhattan, Brooklyn and Queens). This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. The Nurse Practitioner (NP) practices in Primary Care Service, Mental Health Primary Care, Medical, Surgical in both inpatient and outpatient clinical settings to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. The NP practices to the full extent of their education, training, and certification. The NP has prescriptive authority for a panel of patients or program as indicated by their position.

Responsibilities

  • Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
  • Develops and implements an individualized plan to achieve optimal population health and disease management.
  • Demonstrates leadership and works collaboratively with the interdisciplinary team.
  • Orders and interprets labs, tests, and diagnostic studies.
  • Prescribes pharmacologic and non-pharmacologic interventions.
  • Initiates referrals and consults to specialty services.
  • Provides education to patients and their families.
  • Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.

  • Performs comprehensive pain assessments, including evaluation of chronic pain, acute pain exacerbations, and neuropathic, musculoskeletal, or post-surgical pain conditions.
  • Independently manages a panel of complex pain patients using evidence-based, multi-modal treatment strategies.
  • Orders and interprets diagnostic studies relevant to pain including labs, X-rays, CT scans, MRIs, and EMG/NCS.
  • Provides pharmacologic management including non-opioid analgesics, adjuvant agents, and opioid therapy when clinically appropriate.
  • Initiates, monitors, and documents opioid tapers, detoxification, and transitions, following VA/DoD Clinical Practice Guidelines and VISN policies.
  • Prescribes and adjusts non-pharmacologic therapies including PT, CBT for pain, integrative modalities, and self-management tools.
  • Reviews opioid safety metrics including PDMP, urine drug screening, opioid treatment agreements/consent, and STORM reviews.
  • Participates in procedural planning and may perform interventional pain procedures as credentialed and privileged.
  • Works closely with pain physicians, psychology, physical therapy, pharmacy, primary care, and mental health to deliver coordinated, team-based care.
  • Provides consultation to other services regarding pain management strategies, medication safety, and risk mitigation.
  • Educates patients and families regarding pain conditions, treatment options, medication use, and safe pain self-management.
  • Provides education for nursing, trainees, and allied health on best practices in pain care.
  • Participates in quality improvement activities related to pain management and ensures compliance with accreditation and regulatory standards.
  • Completes accurate and timely documentation of clinical encounters, medication changes, registries, and safety reviews in accordance with VHA guidelines.
  • Reports adverse events and participates in clinical review processes as required.
