About The Position

The Advanced Practice Nurse is a licensed independent practitioner with advanced education and national certification who will provide evidence-based care for a population of patients. The Nurse Practitioner will provide a full range of services emphasizing quality, timeliness, and the advancement of the Veteran Affairs (VA) mission. This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until May 29, 2026 , or until positions are filled, closed, or canceled. The Advanced Practice Nurse (APN) will practice in the Primary Care Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The Nurse Practitioner will provide advanced clinical practice, consultation, and management. The Nurse Practitioner practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The Nurse Practitioner prescriptive authority for a panel of patients or program as indicated by their position.

Requirements

Responsibilities

  • Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
  • Develop and implement an individualized plan to achieve optimal population health and disease management.
  • Demonstrate leadership and work collaboratively with Primary Care Team and other interdisciplinary team members.
  • Order and interpret labs, tests, and diagnostic studies.
  • Prescribe pharmacologic and non-pharmacologic interventions.
  • Initiate referrals and consult to specialty services.
  • Provide education to patients and their families.
  • Assess and adjust the plan of care as needed to achieve optimal patient outcomes.
