About The Position

The Nurse Practitioner (NP) in Primary Care at the Veterans Health Administration is responsible for providing comprehensive healthcare services to a diverse population of veterans. This role emphasizes quality care, health promotion, and disease prevention, allowing the NP to practice to the full extent of their education and training. The NP will manage a panel of patients, collaborate with interdisciplinary teams, and ensure optimal patient outcomes through evidence-based practices.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
  • Must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP.
  • Must maintain full and current board certification in an area of clinical specialty.
  • Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner from an accredited institution.

Responsibilities

  • Independently manage a panel of patients with acute and chronic health conditions.
  • Develop and implement individualized plans for optimal population health and disease management.
  • Demonstrate leadership and collaborate with the Primary Care Team and other interdisciplinary team members.
  • Order and interpret labs, tests, and diagnostic studies.
  • Provide timely follow-up on results and complete view alerts.
  • Perform and record history and physical examinations.
  • Enter progress notes in computerized medical records and complete workload documentation.
  • Prescribe pharmacologic and non-pharmacologic interventions.
  • Initiate referrals and consults to specialty services.
  • Provide education to patients and their families.
  • Assess and adjust the plan of care as needed to achieve optimal patient outcomes.
  • Consult with other specialties as appropriate.
