Nurse Practitioner – Post-Discharge In-Home Care (New Albany, MS)
About The Position
Segue Health, a faith-based medical practice, is seeking an experienced and compassionate Nurse Practitioner to join our team serving patients in New Albany, Pontotoc, and surrounding areas. This role focuses on providing in-home post-discharge care management and medical treatment for patients transitioning from acute care facilities back to their homes. Requirements
- Current Mississippi medical license and active Medicare provider number required.
- Nurse Practitioner certification in good standing.
- Ability to travel within a 35-mile radius from home to see patients throughout New Albany and Pontotoc areas.
- Strong communication, clinical judgment, and documentation skills.
- A compassionate, faith-driven approach to patient care.
Responsibilities
- Conduct in-home patient visits to assess clinical status and provide post-discharge medical care.
- Review and interpret discharge summaries and progress notes.
- Collaborate closely with home health professionals and other members of the care team to ensure seamless coordination of care.
- Provide medical consultations, prescribe medications when appropriate, and oversee ongoing care plans to promote recovery and reduce readmissions.
Benefits
- Base Salary: $115,000 – $135,000 (commensurate with experience)
