About The Position

The Nurse Practitioner/Physician Assistant will work in the Primary Care department, providing comprehensive care in a primary care setting. This full-time position may require varying hours, including evenings and weekends, and involves shared on-call responsibilities with other providers (1:4). The role is based at 6374 Lincoln Ave, Chicago, IL 60659, and reports to the Manager of Clinical Operations and the Chief Medical Officer (CMO). The compensation for this position ranges from $98,000 to $115,000.

Requirements

  • Minimum 2 years' experience in the field, ideally in a Federally Qualified Health Center (FQHC) or community health center.
  • Proven experience in both adult and pediatric medicine.
  • Master's degree in Nursing or Physician Assistant Studies from an accredited institution.
  • Current state licensure as a Nurse Practitioner or Physician Assistant.
  • Certification in Family Medicine (preferred) to encompass both adult and pediatric patient populations.
  • Graduate of an accredited school (PA, NP).
  • Current license.
  • Current provider card in CPR.
  • Basic computer skills.
  • Familiarity with or capability of learning electronic health records.
  • Current DEA and Controlled Substance License when applicable.
  • Ability to respond to healthcare emergencies.
  • Strong communication skills and ability to work in both superior and subordinate roles.

  • Certification in Family Medicine.

Responsibilities

  • Deliver care in a primary care setting.
  • Guide and assist in all aspects of primary care medicine including preventative medical care, psychosocial needs, acute medical care, diagnostics, treatment, consultation, and patient education.
  • Perform age-appropriate history and physical patient exams.
  • Engage in care for acute and chronic illnesses.
  • Recognize critical illness and notify the appropriate entities.
  • Collaborate with supervising physician as needed.
  • Prescribe appropriate pharmacologic and non-pharmacologic treatments.
  • Implement interventions to restore patient's desired physiologic status.
  • Monitor effectiveness of interventions.
  • Diagnose common and less common ailments.
  • Investigate clinical problems and offer sound medical solutions.
  • Communicate effectively with other healthcare team members, patients, family members, and healthcare designees.
  • Perform procedures utilizing medically approved approaches and equipment appropriate to the patient's age and size.
  • Provide patient education at a level appropriate to the comprehension of the patient and/or significant others.
  • Actively participate in Quality Improvement activities.
