A well-established outpatient clinic in Chino Valley, AZ is seeking a full-time Nurse Practitioner or Physician Assistant to join its dedicated primary care team. Requirements

  • Current and unrestricted license in Arizona as a Nurse Practitioner or Physician Assistant
  • DEA registration or eligibility to obtain in Arizona
  • Clean professional background (no disciplinary actions or malpractice claims)
  • Credentialing eligibility with Medicare, Medicaid, and commercial insurers
  • AZ State NP/PA License (Or in process)
  • NP/PA Program Completed in USA (Not Puerto Rico)
  • Active Unrestricted DEA License

Nice To Haves

  • Experience in outpatient primary care or family medicine preferred

Responsibilities

  • Perform wellness exams, physicals, and preventive screenings
  • Diagnose and treat acute and chronic medical conditions
  • Create and manage personalized care plans
  • Order, interpret, and follow up on labs and imaging
  • Educate patients on lifestyle changes and disease prevention
  • Collaborate with a medical scribe for accurate clinical documentation
  • Coordinate referrals and manage care transitions
  • Uphold clinical quality standards and best practices
