About The Position

Saratoga Hospital Medical Group - Geriatric Medicine seeks a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Geriatric Medicine team. The APP will provide consultative care for older adults across diverse care settings, primarily in an outpatient consultative role but the role may also consist of inpatient consults, visits to assisted living facilities, and home visits. In close collaboration with an established geriatrician, the APP will deliver patient-centered care that prioritizes quality of life, value-based decision-making, and collaborative goal-setting with patients and caregivers.

Requirements

  • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) with prescriptive authority.
  • Licensure in New York State prior to commencement of credentialing.
  • Strong interpersonal and communication skills with patients, families, and interdisciplinary teams.
  • Demonstrated expertise in geriatric syndromes, polypharmacy, functional assessment, and cognitive evaluation.
  • Comfort with home visits, assisted living consultations, and transitions of care.
  • Commitment to patient-centered and value-based care models.

Nice To Haves

  • Certification and clinical experience in geriatrics, internal medicine, family practice, or related specialty strongly preferred.

Responsibilities

  • Provide comprehensive geriatric assessments, including history, physical exam, and review of functional, cognitive, and psychosocial health.
  • Provide falls assessments, polypharmacy identification and, when appropriate, deprescription plans, cognitive assessments and dementia care.
  • Conduct longer visits that allow for thoughtful, patient- and family-centered care.
  • Manage acute and chronic medical conditions in older adults with an emphasis on appropriate workup and diagnosis aligned with patient goals and values.
  • Elicit and document goals of care, advance care planning, and shared decision-making conversations with patients and caregivers.
  • Perform consultative evaluations in the hospital, outpatient clinic, assisted living facilities, and patient homes as needed.
  • Coordinate care across healthcare settings, collaborating with primary care providers, specialists, caregivers, and community resources.
  • Participate in interdisciplinary team meetings to optimize care plans for medically and socially complex patients.
  • Communicate effectively and compassionately with patients, families, and healthcare teams.
  • Ensure documentation and care planning are consistent with regulatory standards and value-based care principles.
  • Collaborate with supervising geriatrician for case discussions, clinical oversight, and professional growth.
