About The Position

Albany Family Medicine (AFM), part of Community Care Physicians (CCP) — the Capital Region’s first and largest independent multispecialty medical group — is seeking a Nurse Practitioner or Physician Assistant to join our dedicated primary care team. AFM is a cornerstone of the Albany community and is proud to serve as a teaching practice with an established Family Medicine Residency Program. Our practice blends compassionate, relationship-based care with a strong commitment to education, collaboration, and continuous learning. We care for patients across the lifespan and value the important role Advanced Practitioners play in both patient care and the academic mission of the practice. As an Advanced Practitioner at Albany Family Medicine, you will provide comprehensive family medicine care in a supportive, team-based environment that includes attending physicians, residents, nurses, and clinical support staff. You will practice to the full extent of your license, supported by CCP’s extensive specialty network and resources. This position offers no after-hours call, allowing you to maintain a healthy work–life balance while focusing on delivering high-quality, patient-centered care. Albany Family Medicine is home to a Family Medicine Residency Program, fostering a culture of learning, mentorship, and collaboration. Advanced Practitioners are valued members of the care team and play an important role in supporting resident education while maintaining continuity of patient care.

Requirements

  • Current New York State NP or PA license.
  • National certification (or eligibility).
  • Strong clinical, communication, and teamwork skills.

Responsibilities

  • Provide primary care services for patients of all ages, including acute, chronic, and preventive care
  • Perform patient assessments, diagnose conditions, and develop individualized treatment plans
  • Order, interpret, and follow up on laboratory tests and diagnostic studies
  • Prescribe medications and manage ongoing treatment plans
  • Educate patients and families on wellness, prevention, and disease management
  • Collaborate closely with physicians, residents, and interdisciplinary team members
  • Support a teaching environment by working alongside and contributing to the education of family medicine residents
