About The Position

We’re hiring a full-time Nurse Practitioner or Physician Assistant to provide ongoing care within post-acute and long-term care settings across Mesa and Chandler.
This position offers steady weekday hours, autonomy, and a manageable patient load in a supportive clinical environment.
Providers typically see 15–16 patients per day in Skilled Nursing and Assisted Living communities, focusing on established, stable residents and new admissions as needed.

Requirements

• Active, unrestricted Arizona NP or PA license
• Graduate of an accredited U.S.-based NP or PA program
• Strong communication, clinical reasoning, and organizational skills
• Reliable, professional, and comfortable working independently
• Current and unrestricted Arizona Nurse Practitioner or Physician Assistant license
• National certification (AANP, ANCC, or NCCPA)

Nice To Haves

• 1–2 years of post-acute, long-term, or primary care experience

Responsibilities

• Provide direct, in-person care at assigned SNF and ALF facilities
• Maintain an on-site presence daily
• Participate in weekend rotation once or twice per month
• Manage 15–16 patient visits per day, including new and follow-up appointments
• Respond to nursing staff calls related to your assigned patients
• Conduct assessments, adjust medications, and manage chronic conditions
• Collaborate with facility staff and interdisciplinary teams
• Document all encounters accurately using PointClickCare (PCC) EMR
