About The Position

As an Advanced Practice Provider, you will play an integral role in building and expanding a virtual primary care program. You’ll be a part of the ambulatory care teams by providing high-quality, cost-effective care to patients both in person and virtually. We are seeking a Nurse Practitioner OR Physician Assistant to: Be a part of a virtual care team expanding access to primary care through telehealth visits Care for the acute and chronic needs of patients from newborn to elderly. Perform physical exams and manage the health maintenance needs of our family medicine population. Perform routine clinical procedures independently including wart removal, PAP smears and skin lesion removals.

Requirements

If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program.
Two (2) years of relevant clinical experience
Minimum – State of Wisconsin NP or PA licensure
Active DEA license or ability to apply for such license prior to or upon hire
If Nurse Practitioner: Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND Active board certification as a NP in area of clinical practice. These may include the following: Family.
If Physician Assistant: Active PA license, including prescriptive authority, in state(s) of practice AND Active PA board certification

Responsibilities
