Requirements

Active or eligible Arizona NP or PA license
Graduate of an accredited NP or PA program in the U.S.
(Note: Puerto Rico-based programs are not eligible)
DEA registration (or ability to obtain before start date)
Strong clinical decision-making and interpersonal skills

Nice To Haves

Minimum one year of experience in outpatient family medicine

Responsibilities

Provide comprehensive outpatient care to pediatric, adult, and geriatric patients
Diagnose and manage chronic conditions such as hypertension, diabetes, and asthma
Perform annual physicals, preventive health screenings, and wellness visits
Order, interpret, and follow up on labs, imaging, and other diagnostics
Develop individualized treatment plans in collaboration with patients and care teams
Counsel patients on preventive health, lifestyle changes, and medication adherence
Coordinate specialist referrals and ensure appropriate follow-up
Document encounters accurately and promptly in the EHR
Participate in care coordination and quality improvement initiatives
