NP/PA PT (20hrs) position, 10-hour shifts, mostly weekdays, occasional weekends and holidays per Division policy. Exciting opportunity to join an adult Palliative Care inpatient practice in a large academic medical center. Our service attends upon patients admitted to the Palliative Care Unit (4-1200), usually for end-of-life care, and consults elsewhere in the hospital on patients with serious illness at all stages, to: 1. help manage complicated symptoms, 2. facilitate care goals conversations, and 3. provide support to patients, families and colleagues. The APP’s primary responsibility is the collaborative care of patients and families, to include consultative and admission work-ups, follow-up visits, team rounding, EHR chart documentation, order writing on 4-1200 patients, and discharge planning. Other vital responsibilities include participation in clinical and educational conferences and rounds, APP student precepting, multidisciplinary trainee teaching and support, and support of 4-1200 and other hospital Units’ nurses, clinicians and staff. The ideal applicant is committed to the palliative care mission, possesses excellent clinical and communication skills, is highly motivated to deepen their palliative care knowledge and skills, is both independent and committed to interdisciplinary collaboration, and is committed to diversity, equity and inclusion. The candidate must be NYS eligible for NP or PA certification. We are not accepting applications from new graduates for this position.
