Practice locations can include Duke Health’s three hospitals or over 140 primary care and specialty care clinics throughout North Carolina.     Job Location      Brier Creek Infusion Center, 10207 Cerny Street, Ste 200, Raleigh, NC South Durham Infusion Center, 234 Crooked Creek Pkwy, Ste 400, Durham, NC Duke Health Cary Place Clinic, 100 Duke Health Cary Place, Cary, NC   Job Summary We are seeking an Advanced Practice Provider in our Ambulatory Specialty Infusion Center to work collaboratively with the medical, nursing and pharmacy teams to provide direct patient care services for patients.  Clinic locations will include the Brier Creek Infusion Center and the South Durham Infusion Center, and the Duke Health Cary Place Clinic.  There will be rotation at all of the sites.   Position hours are Monday-Friday, four days per week, beginning at 7:40am and ending when infusion clinic closes at 6pm or when the last patient leaves.  The day off during the week may vary based on scheduling needs.  This position could include Saturday shifts with supplemental pay once every 8 weeks.  The APP will also collaborate with fellow APP team members and cross-cover when needed.  This is primarily to cover APP vacation and time-off schedules.

Requirements

  • Nurse Practitioners:  Work requires completion of an accredited Nurse Practitioner program, with a Master's degree in Nursing.
  • Physician Assistants:  Work requires completion of an accredited Physician Assistant program. A Bachelor's degree required; Master's degree strongly preferred.
  • Must have current or compact RN licensure & NP approval to practice in the state of North Carolina. BLS required. National certification as a NP required.
  • Must be licensed in the state of NC, and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants. BLS required.
  • ACLS certification required (candidate may obtain this certification during onboarding)
  • BLS required.

Responsibilities

  • In collaboration with attending physician(s), the NP/PA performs triage, physical exams, collects and documents data, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, coordinates the care of patients, and provides direct patient care services.
  • Analyze clinical practice patterns so as to provide the best medical care and to increase effectiveness and efficiency.
  • Communicate verbally and in writing all pertinent patient information to supervising MD, nursing staff, patient resource manager, home health agency, and other relevant healthcare providers.
  • Provide timely documentation in the electronic medical record of all relevant data including results of diagnostic tests; laboratory results; patients’ condition and response to therapies/interventions; communications with physician colleagues.
  • Update therapy plans per changes in service line provider preference and to reflect best practice at Duke
  • Update therapy plans per provider request and send via in basket for signature
  • Assist providers with order entry and review. Reviews in advance all orders for South Durham, Brier Creek, and Cary Clinics to confirm dosing and scheduling accuracy along with accuracy of referrals. Referral review and diagnosis accuracy to ensure no more than one diagnosis is associated with the therapy plan.
  • Update same day pre-medications, especially in anticipation of patients who have had a prior hypersensitivity reaction.
  • Present during all hypersensitivity reactions, assist with patient recovery, order entry, and how to disposition the patient following (i.e., may be discharged to home setting or need to be admitted to the ED).
  • Resource for same day questions, i.e., medication orders, therapy plans and necessity for update and review
  • Place same day notes of patients who necessitate "ok to treat" order and detail the clinical situation in form of short version of SOAP note.
  • Update medications, add medications that have "fallen off therapy plans"
  • Assist in transfers of care to ED.  Communicate directly with referring provider in instances that warrant immediate attention
  • Provides education/in-services on therapy plan entry to service lines.  Work with Epic as a super user for therapy plan design
  • ACLS certification required (candidate may obtain this certification during onboarding).
  • Four-day work week would require APP to remain on site during the 10-hour shifts.  Non-work appointments should be scheduled on the APP’s day off each week.
