About The Position
As an Advanced Practitioner at CapitalCare Family Practice Slingerlands, you will provide full-spectrum family medicine care alongside experienced physicians and Advanced Practice Providers. The practice emphasizes preventive medicine, chronic disease management, and strong patient relationships, with modern tools such as telemedicine and a secure patient portal to support coordinated care and patient access.
Responsibilities
- Conduct comprehensive patient evaluations and manage acute and chronic conditions
- Order and interpret diagnostic studies, prescribe medications, and coordinate specialty referrals as needed
- Provide preventive care, wellness visits, and patient education across the lifespan
- Collaborate closely with physicians, Advanced Practitioners, nurses, and support staff to ensure continuity of care
- Maintain accurate and timely clinical documentation in accordance with organizational standards
