About The Position

This is a multi-site clinical position designed for an NP or PA who wants to practice at the top of their training in a supportive, collaborative environment. You will be integrated into the physician’s practice — not functioning independently without support. This role offers direct mentorship with physicians who enjoy teaching and case discussion, providing active collaboration, real-time feedback, and meaningful clinical development. It features a true continuity model where you will see patients in clinic, assisted/independent living, skilled nursing, and after hospital discharge, offering rare exposure to the full arc of geriatric care. The position involves managing complex medicine cases such as polypharmacy, frailty syndromes, dementia care, high-utilizer patients, advanced care planning, and chronic disease optimization. For those interested, a long-term leadership trajectory may include involvement in clinic management, clinical operations strategy, program expansion, and interdisciplinary team development. The role matters because our aging population deserves clinicians who understand the complexity of late-life medicine, offering the opportunity to build deep relationships with patients and practice thoughtful, high-quality geriatric care in a supportive environment.
