We are seeking a full-time Nurse Practitioner or Physician Assistant to join our community-focused outpatient family medicine clinic in Dewey-Humboldt, AZ. Requirements

  • Active and unrestricted Arizona NP or PA license
  • Current DEA registration or ability to obtain
  • Credentialing eligibility with Medicare, Medicaid, and commercial insurers
  • No history of malpractice or disciplinary action
  • Graduate of an accredited U.S.-based NP or PA program
  • (Note: Puerto Rico-based programs are not accepted)
  • AZ State NP/PA License (Or in process)
  • NP/PA Program Completed in USA (Not Puerto Rico)
  • Active Unrestricted DEA License

Responsibilities

  • Perform annual wellness exams, preventive screenings, and Medicare visits
  • Diagnose and manage acute conditions and minor injuries
  • Provide chronic disease management (diabetes, hypertension, COPD, etc.)
  • Order, interpret, and follow up on diagnostic tests and imaging
  • Educate patients on medications, treatment plans, and healthy lifestyle practices
  • Maintain accurate EHR documentation with synchronous scribe support
  • Coordinate referrals and manage transitions of care
  • Participate in clinical quality initiatives and practice improvement efforts
