What you will do: This position is responsible for rendering professional medical services to the patient population of Delaware Valley Community Health, Inc. This position augments the Physician's data gathering abilities and assists in reaching decisions and instituting care plans for our patients. The position is also accountable for the complete documentation of patient visits in the patient chart as well as administrative reporting activities as assigned by management. This role will require travel to and from the 9 health centers operated by DVCH. DVCH is an EOE/Veterans/Disabled/LGBTQ+ employer. Requirements

  • Registered Nurse Practitioner or Physician Assistant in the Commonwealth of Pennsylvania is required.
  • Excellent interpersonal and oral and written communications skills are required.

Nice To Haves

  • Previous experience in Adult/Family Practice is highly preferred.
  • Bilingual (English/Spanish) preferred.

Responsibilities

  • Act as an advanced care practitioner and assist in coordinating all aspects of patient care.
  • Responsible for rendering professional medical services to the patient population.
  • Accountable for the complete documentation of patient visits in the patient chart and administrative reporting activities assigned by management
  • Provide health and wellness advice to patients or caregivers.
  • Completes other duties as assigned.
