About The Position

The Per Diem Advanced Practice Nurse (APN) or Physician Assistant (PA) for the Transition Utilzation Review Team utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN or PA works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Minimum of 1 holiday per year.

Requirements

  • Graduate of an NLN/AACN accredited nursing program.
  • Master's or Doctorate degree from an accredited Nurse Practitioner program.
  • Maintains clinical competencies through continuing education and clinical practice.
  • Experience in a relevant patient care setting (or as regulations dictate).
  • National Provider Identifier (NPI).
  • Prescriptive authority through CDS and DEA registration according to New Jersey state law.
  • Collaborative agreement with physicians on staff.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
  • Successful completion of an approved physician's assistant program, accredited by the AMA Council on Medical Education.
  • NJ State Professional Registered Nurse License.
  • Advance Practice Nurse License.
  • BLS AHA Basic Health Care Life Support HCP Certification.
  • Board certified by AANP or ANCC.
  • Physician Assistant License.
  • National Commission on Certification of Physician Assistants.

Nice To Haves

  • Advanced Cardiac Life Support Certification.
