Job Summary: The Nurse Practitioner or Physician Assistant will provide comprehensive primary care services in an outpatient family medicine setting. This role includes delivering preventive, acute, and chronic care to a diverse patient population while supporting quality initiatives and coordinated, team-based care. The provider will maintain accurate documentation and contribute to ongoing practice-wide quality improvement efforts.

Requirements

  • Board Eligible or Board Certified Nurse Practitioner or Physician Assistant
  • Valid Arizona medical license (or ability to obtain)
  • Current DEA registration
  • Experience in outpatient primary care preferred
  • Strong patient communication and interpersonal skills

Responsibilities

  • Perform annual wellness exams and provide preventive, acute, and chronic care
  • Manage a diverse panel of patients across all ages
  • Maintain accurate and timely documentation in the EMR
  • Collaborate with clinical staff to ensure coordinated, patient-centered care
  • Support managed care initiatives and quality programs
  • Provide patient education to promote healthy lifestyle and disease prevention
  • Participate in practice-wide quality improvement efforts
