About The Position

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Maple Grove and Plymouth, MN. Our unique, robust model of care goes beyond primary care services — our multidisciplinary care teams collaborate with patients, their families and other healthcare providers to deliver care that is preventative, proactive and tailored to their unique needs. Under our model of care, Bluestone patients experienced 21% fewer ER visits, 36% fewer hospitalizations and 41% fewer hospital readmissions compared to patients with similar conditions and complexities over the same time period. Position Overview: Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.) Ability to design and manage your schedule Develop and implement care plans appropriate to patients and their desires Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers Access to diagnostics such as labs and x-rays as needed Collaborate with community partners within home care, hospice, and other resources as appropriate

Requirements

  • Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
  • Board Certified/Board Prepared through AANP, ANCC, or NCCPA
  • Licensed or ability to obtain licensure through the Minnesota Board of Nursing or Board of Medical Practice
  • Ability to obtain and /or maintain DEA certification for the State of Minnesota
  • Proven ability to communicate effectively with strong verbal skills
  • Excellent interpersonal and customer service skills
  • Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required
