Requirements

A NP or PA with a master’s degree and active Colorado licensure
Passionate about serving seniors and medically complex populations
Skilled at building trust, communicating clearly, and adapting to home-based environments

Responsibilities

Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and facilities.
Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.
Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
