About The Position

The Palliative Care NP/PA serves as an advanced practice provider within Hospice of Humboldt’s home-based palliative care program. Working independently and in collaboration with the interdisciplinary team, the NP/PA delivers comprehensive, patient-centered care focused on symptom management and quality of life. Responsibilities include conducting assessments, developing and updating care plans, completing regulatory visits (intake, follow-up, recertification), and facilitating transitions to hospice when appropriate.

Requirements

  • Master’s Degree in Nursing from an accredited college or university
  • Active NP and RN licensure in California
  • Bachelor’s degree or above from an accredited college or university.
  • Graduate of a PA program accredited by the Accreditation Review Commission on Education for the Physician Assistant, Inc.
  • Active PA licensure in California
  • Minimum two years’ experience as an advanced practice provider.
  • Strong clinical assessment skills.
  • Commitment to interdisciplinary collaboration.
  • Compassionate and patient-centered approach to care.
  • Excellent communication and counseling skills with patients, families, and interdisciplinary teams.
  • Ability to work independently while fostering collaboration.
  • Proficiency with electronic medical records and basic computer systems.
  • Organizational and time-management skills to prioritize tasks and meet deadlines.
  • Valid driver's license and reliable transportation.

Nice To Haves

  • Prior palliative care experience.
  • PAs: Certification as a PA by the National Commission on Certification of Physician Assistants (NCCPA)
  • NPs: National Board Certification as a NP from an agency accredited by the American Board of Nursing Specialties (ABNS).

Responsibilities

  • Evaluates and treats patients with acute, chronic complaints and health maintenance concerns
  • Perform comprehensive medical, functional, and psychosocial assessments
  • Prescribe, adjust, and discontinue medications (including opioids) in accordance with standardized procedures or physician orders.
  • Conduct intake, follow-up, and recertification visits for palliative care patients.
  • Develop and revise individualized care plans in collaboration with the interdisciplinary group (IDG); recommend treatment options and monitor outcomes.
  • Provide education and counseling to patients and families regarding diagnoses, prognosis, goals of care, and treatment choices.
  • Support families in decision-making related to interventions, resuscitation status, and end-of-life planning.
  • Participate in interdisciplinary team meetings and serve as a liaison with physicians, discharge planners, and other providers.
  • Deliver education to staff, nursing facility teams, and families on hospice and palliative care principles.
  • Contribute to the development and implementation of evidence-based protocols and educational materials.
  • Maintain accurate documentation in the electronic health record (EHR) and meet productivity standards.
