Wentworth-Douglass Hospital is a not-for-profit health care organization in Dover, New Hampshire, with a 110-year history of compassionate care and innovation. Serving the community since 1906, it is made up of 3,000 employees, including 500+ registered nurses and more than 480 physicians and advanced practitioners, dedicated to the health, safety, and well-being of the residents and visitors to the Seacoast area of New Hampshire and Southern Maine. Our award-winning Supportive & Palliative Care team addresses both the physical symptoms and the stress and worry that accompany a serious illness for both the patients and their families and loved ones. Many adults living with illnesses such as cancer, heart disease, lung disease, kidney failure, dementia, among others, experience physical symptoms and emotional distress related to their diseases. Unlike hospice care, patients do not need to have a terminal diagnosis to be referred for palliative care services. Many patients experience shorter hospital stays, less pain and improved overall well-being, and the research backs this up. Palliative care provides patients with relief from distressing symptoms including pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, problems with sleep and many other symptoms. It can also help patients deal with the side effects of the medical treatments they’re receiving with the goal of improving overall quality of life. In addition to symptom management, our team also supports patients with the following: • Advanced care planning: guidance with difficult treatment decisions, help with navigating the health care system, assistance with Advanced Directives • Strong physical, emotional and spiritual support throughout all stages of illness • Supportive counseling: clear and compassionate communication with doctors and family members • Coordination of care with various community services A team-based approach is central to palliative care including doctors, nurse practitioners and nurse coordinators who specialize in palliative care, social workers, chaplains, nutritionists, counselors and others. Our team meets with patients and families during their hospital stays, in the Mass General Cancer Center at Wentworth-Douglass Hospital, in our outpatient clinic, and via virtual visits. We are seeking a Per Diem Nurse Practitioner to support our Supportive & Palliative Care team. We are located at 789 Central Avenue, Dover, New Hampshire 03820. The Nurse Practitioner will provide coverage for our full-time NPs during planned vacations, sick time, and as they fulfill various continuing education requirements ideally between the hours of 8:00am to 4:00pm, Monday-Friday. As true partners to our physicians, our Nurse Practitioners get the opportunity to work autonomously to the top of their education/certification while also functioning as part of the collaborative team. The Nurse Practitioner is responsible for providing exceptional direct diagnostic and therapeutic patient care to the highest standards of the organization as part of a multidisciplinary team. The Nurse Practitioner participates in the delivery of patient care in the outpatient setting by performing initial and follow-up visits, as well as autonomous performance of approved clinical functions. The Nurse Practitioner cultivates a patient centric environment which focuses on the whole individual inclusive of physical, psychosocial, spiritual and functional needs of the patient and family. The NP acts as a resource and collaborates with clinical staff and other hospital and health care disciplines for promotion of health to the optimal level of wellness for patients, families, and the community.
