The Palliative Care Nurse Practitioner at South Shore Health plays a crucial role in providing comprehensive care for patients with advanced and progressive medical conditions. Working closely with a multi-disciplinary team, the Nurse Practitioner is responsible for conducting initial and ongoing clinical assessments for patients referred for palliative care consultations, which can occur in both inpatient and outpatient settings. This position emphasizes symptom management, decision-making assistance, and the establishment of goals of care, ensuring that patients receive holistic support throughout their treatment journey. The Nurse Practitioner will collaborate with attending and consulting physicians to modify care plans as necessary, ensuring that the patient's needs are met effectively. In addition to clinical responsibilities, the Nurse Practitioner will serve as a clinician educator, contributing to various educational initiatives within the South Shore community. This role is vital as the Palliative Care Service is expanding, and the Nurse Practitioner will be expected to participate in night telephone calls and weekend coverage as the service develops. The ideal candidate will possess a strong background in palliative care, demonstrating compassion and competence in managing complex patient needs. The position requires a proactive approach to care coordination, emotional and spiritual support, and bereavement assistance, ensuring that patients and their families receive comprehensive care until discharge or death.
