About The Position

The Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist at Wooster Express Care II is a nationally certified Advanced Practice Provider responsible for delivering primary and specialty care in a fast-paced healthcare environment. This role emphasizes health promotion, disease prevention, and the management of acute and chronic health issues, while collaborating with a multidisciplinary healthcare team. The position requires flexibility to float between departments and involves providing comprehensive care to patients aged six months and older, with a focus on ambulatory care.

Requirements

  • Nationally certified as an Advanced Practice Provider.
  • Master's degree in Nursing or Physician Assistant Studies from an accredited program.
  • Currently licensed as a Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist in applicable state(s).
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification may be required based on work setting.
  • DEA Certification for prescribing medications.

Nice To Haves

  • 1 year of advanced practice experience.
  • Ambulatory care experience, preferably in primary care.
  • Family certification for Nurse Practitioners.

Responsibilities

  • Conduct thorough medical histories and perform complete physical examinations.
  • Initiate appropriate lab, radiology tests, or other special tests required for evaluation of illness.
  • Scrutinize lab data to establish a record of the patient's current health status and develop a working diagnosis and treatment plan.
  • Work in collaboration with the healthcare team.
  • Perform and interpret common laboratory, radiologic, cardiographic, and other routine diagnostic procedures.
  • Prescribe and monitor medications as allowed by license.
  • Educate patients about their disease, treatments, and related drug side effects.
  • Participate in medical team rounds and collaborate with nursing and other healthcare team members.
  • Refer patients to specialists as appropriate for consultation or specialized health resources.
  • Ensure continuity of care by serving as a liaison between patients and other members of the care team.
  • Participate in quality monitoring through the review of records and treatment plans for patient outcomes.
  • Facilitate appropriate length of stay, discharge planning, and compliance with regulatory standards.
  • Support professional development of the healthcare team and promote evidence-based practices.
