About The Position

Cleveland Clinic Health System is seeking a motivated and qualified Physician Assistant, Clinical Nurse Specialist, or Nurse Practitioner for a full-time position with the Pulmonary Vascular Program. This provider will oversee the care of adult, high-risk patients with pulmonary hypertension, working within an interprofessional team of physicians, RNs, and coordinators. The role offers the unique opportunity to facilitate the transition of care from inpatient to outpatient settings and encompasses efficient inbasket management, ensuring tasks, messages, and requests are handled promptly. The hours for this position will be Monday through Friday, with flexibility based on departmental need. New graduates are welcome to apply, although candidates with cardiology, cardiothoracic surgery, or pulmonary experience/rotations are strongly preferred.

Requirements

  • Nationally certified Advanced Practice Provider (Nurse Practitioner, Clinical Nurse Specialist, or Physician Assistant).
  • Graduate of an accredited APRN program or A.R.C. approved Physician Assistant Program.
  • Currently licensed as a Nurse Practitioner, Clinical Nurse Specialist, or Physician Assistant in applicable state(s).
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) required.
  • Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification may be required based on work setting.
  • Must have arrangements with at least one physician in accordance with state Board of Nursing requirements.
  • Must be credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.

Nice To Haves

  • Experience in cardiology, cardiothoracic surgery, or pulmonary rotations.
  • Related clinical experience preferred.

Responsibilities

  • Conduct thorough medical histories and perform complete physical examinations.
  • Initiate appropriate lab, radiology tests, or other special tests required for evaluation of illness.
  • Scrutinize lab data on new and former clinic patients and hospital admissions.
  • Perform and interpret common laboratory, radiologic, cardiographic, and other routine diagnostic procedures.
  • Perform routine procedures such as injections, immunizations, suturing, and wound care.
  • Assist in the management of complex illness and injury, including assisting surgeons in operations.
  • Prescribe and monitor medications as allowed by prescriptive authority.
  • Provide information and answer routine questions of patients regarding their disease.
  • Educate patients regarding their disease, treatments, and related drug and treatment side effects.
  • Participate in medical team rounds and collaborate with nursing, medical, and other healthcare team members.
  • Refer patients to specialists as appropriate for consultation or specialized health resources.
  • Ensure continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team.
  • Perform research follow-up studies and assist in data analysis.
  • Participate in quality monitoring through the review of records and treatment plans for patient outcomes.
  • Facilitate appropriate length of stay, discharge planning, and compliance with regulatory standards.
  • May serve as clinical preceptor for PA/APRN students and other health professionals.
