The Cleveland Clinic Center for Functional Medicine is looking to add an Advanced Practice Provider who is experienced in Functional Medicine and certified by The Institute for Functional Medicine (IFM). This provider must be able to integrate this patient care approach with the reputation and integrity of the Cleveland Clinic. This Advanced Practice Provider will focus on the Functional Medicine team approach to provide excellent patient experiences and deliver innovative, value-based care at the Cleveland Clinic.

Requirements

  • Functional Medicine Certification through The Institute of Functional Medicine (IFM).
  • Excellent analytical and decision-making skills.
  • Ability to work effectively and efficiently with diverse team members, caregivers, students, and patients.
  • Superior communication skills and emotional intelligence.
  • Demonstrated competency in advanced assessment and diagnosis.
  • Current licensure as a Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist in applicable state(s).
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification may be required based on work setting.

Nice To Haves

  • Related clinical experience preferred.
  • Experience in a multi-disciplinary team environment.

Responsibilities

  • Conduct thorough medical histories and perform complete physical examinations.
  • Initiate appropriate lab, radiology tests or other special tests required for evaluation of illness.
  • Order, interpret and evaluate laboratory tests and other diagnostic studies.
  • Develop a treatment plan in collaboration with the physician and the patient.
  • Educate patients regarding their disease, treatments, and related drug and treatment side effects.
  • Ensure continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team.
  • Participate in quality monitoring through the review of records and treatment plans for patient outcomes.
  • May serve as clinical preceptor for PA students and other health professionals.
