About The Position

The Nurse Practitioner / Physician Assistant position at Alignment Healthcare is designed for advanced practice clinicians to provide care for seniors with complex and chronic health needs. This role involves working within an interdisciplinary team to deliver clinical and home-based patient care, ensuring optimal health outcomes while adhering to cost-effective practices and regulatory compliance. The position emphasizes in-home assessments, patient education, and collaboration with primary care providers.

Requirements

  • Master's degree from an accredited Nurse Practitioner or Physician Assistant program.
  • Active California state Nurse Practitioner or Physician Assistant license.
  • Active RN License and Furnishing number upon hire.
  • NP only: Active Nurse Practitioner Board Certification or Physician Assistant Certification.
  • NPI Number, DEA, and valid BLS certification.
  • Valid California driver license and current automobile insurance.
  • Experience in care of older adult (geriatric) patients preferred.
  • Excellent administrative, organizational, and communication skills required.

Nice To Haves

  • One year of prior clinical or home care experience preferred.
  • Previous EMR experience preferred.

Responsibilities

  • Conduct in-home assessments on health plan members, including comprehensive history and physical exams.
  • Perform medication reviews and appropriate cognitive/depression/safety screenings.
  • Engage in end-of-life care discussions as necessary.
  • Collect laboratory specimens in the home setting when appropriate.
  • Identify diagnoses for active medical management and chronic disease management.
  • Communicate assessment findings to inform the primary care provider of potential gaps in care.
  • Provide education to patients, families, and caretakers, focusing on monitoring and follow-up needs.
  • Utilize community resources for patient referrals.
  • Comply with HIPAA regulations and maintain the security of protected health information.
