About The Position

The Alignment Jump Start Assessment (JSA) program is a physician-led initiative aimed at providing care for Alignment Healthcare patients with complex and chronic care needs. As a JSA Advanced Practice Clinician (APC), you will collaborate with an interdisciplinary team to deliver clinical and home-based patient care, ensuring quality outcomes while adhering to regulations. This role involves assessing, developing, and coordinating care options for patients, primarily those aged 65 and over, through in-home and virtual visits.

Requirements

  • Master's degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program.
  • Active California state Nurse Practitioner or Physician Assistant license.
  • Active RN License and Furnishing number (must be obtained upon hire).
  • Active Nurse Practitioner Board Certification or Physician Assistant Certification.
  • NPI Number, DEA, and valid BLS certification.
  • Valid California driver license and current automobile insurance.
  • Experience in care of older adult (geriatric) patients preferred.
  • Excellent administrative, organizational, and communication skills required.

Nice To Haves

  • Preferred one (1) year of prior clinical or home care experience.
  • Previous EMR experience preferred.

Responsibilities

  • Conduct in-home assessments on health plan members, including comprehensive history, physical exams, and medication reviews.
  • Engage in end-of-life care discussions as necessary.
  • Collect laboratory specimens in the home setting when appropriate.
  • Identify diagnoses for active medical management and chronic disease management.
  • Communicate assessment findings to inform the primary care physician of potential gaps in care.
  • Provide education to patients, families, and caretakers, focusing on monitoring and follow-up needs.
  • Utilize community resources for patient referrals.
  • Comply with HIPAA regulations and maintain the security of protected health information.
