Presbyterian Medical Group seeks a Board Eligible / Board Certified Palliative Care Nurse Practitioner or Physician Assistant to join our established practice in Albuquerque, New Mexico. Practice Highlights: 7-8 consults per day Inpatient coverage only Some hospice care will be a part of this role

Requirements

  • Board Eligible / Board Certified Palliative Care Nurse Practitioner or Physician Assistant
  • Nurse Practitioner - Masters degree in Nursing, New Mexico Nurse Practitioner license, ANCC or AANP certification required.
  • Physician Assistant - Baccalaureate prepared CAHEA Certified Physician Assistant or two (2) years working experience as a Certified Physician Assistant, and licensed by the New Mexico Board of Medical Examiners required.

Responsibilities

  • Collaborates with colleagues in the management of patients, in outpatient settings
  • Offers care to patients with critical and life-limiting illness, focused on assisting them to achieve medical treatment concordant with their goals.
  • Offers expert symptom management to assist patients in achieving maximal quality of life, despite their illness.
  • Builds effective relationships with referring physicians and care teams, strives to improve collaboration centered on the care of these patients.
  • In both Palliative and Hospice settings, works with an interdisciplinary team, offering support to both patients and families.
  • Works with colleagues to improve the quality of care offered patients
  • Serves as a consultant in the field of Palliative Medicine, in both inpatient and outpatient settings.
  • Serves as a hospice provider, offering clinical oversight on care of hospice patients, both inpatient and outpatient.
  • Assists Hospice Medical Director with documentation for certifications to obtain hospice benefits for patients.
