About The Position

The LifeCircles clinical team delivers primary care, palliative care, and end-of-life care for patients aged 55 and older. This position is perfect for the healthcare provider who wants to be patient-centered, creative, and collaborative in their approach to care. The successful candidate will join an experienced Interdisciplinary Team and work closely with other Advanced Practice Providers and healthcare professionals in a well-supported, team-based environment.

Requirements

Current license and certification to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Michigan
Master's degree in nursing or satisfactory completion of a physician assistant program
Active Registered Nurse (RN) license in the state of Michigan.
CPR certification or willingness to obtain
Degree in Nursing; bachelor's degree in nursing (BSN) preferred
Minimum of 1 year of nursing experience.
Current DEA registration
State of Michigan Controlled Substance License
Minimum of 1-year experience with the frail and elderly or long-term care population preferred

Nice To Haves

Previous experience in home health, case management, or a related field preferred.

Responsibilities

Participating in daily interdisciplinary team meetings
Completing periodic health assessments
