Nurse Practitioner- Palliative Home Hospice - this role involves seeing patients anywhere in our service territory PT8 or 64 hrs. per two week pay period 8am - 5pm Potential for "on call" in the future Essential Functions Assists with the provision of palliative medicine in accordance with organizational, state, and federal regulations. Serves as a comprehensive direct care provider with demonstrated skilled proficiency in symptom management and advanced illness care. Provides critical thinking and clinical judgment to address the physical, emotional, psychological, and spiritual needs and reflect the goals of care of the patients and families receiving palliative medicine services. Provides clinical direction and expertise to the interdisciplinary team. Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted Choices’ standards and guidelines. Provides comprehensive clinical and psychosocial evaluations of patients and their families in relation to their goals of care and serves as a patient advocate in the development of patient centered goals of care. Collaborates and maintains communication with the patients’ attending providers and sub-specialists for the coordination of palliative medicine services. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities and monitors the effectiveness of interventions. Provides cross coverage to hospice services on an as needed basis including in person visits. Qualifications

Requirements

  • Certified Nurse Practitioner license in the Commonwealth of Pennsylvania required
  • Prescriptive authority required
  • Unrestricted DEA license required
  • Certification in Hospice & Palliative Medicine required within three years from date of hire
  • Must work a minimum of 64 hours per two-week pay period
  • Must be vaccinated for influenza or be granted a religious/medical exemption.

  • Hospice and/or Palliative Medicine experience preferred
