About The Position

Provides comprehensive direct patient care in collaboration (yet able to function in their role independently) with Medical Director and/or patient’s actively involved physicians and with nursing staff. This includes case management, history and physical assessment, diagnosis, therapeutic interventions for relief of pain and symptoms and education of patient, care giver(s) and family. Acts as a clinical resource, role model, educator promoting quality palliative care and integration of services throughout the health care continuum from hospital/health system to community health partners. Promotes and secures adherence to state and regulations and licensing requirements. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Requirements

  • RN licensed in the state of Michigan.
  • Basic Life Support CPR certification.

Nice To Haves

  • Preferred minimum of two years RN, NP, or APN home care, geriatrics or Hospice/Palliative Medicine experience; preferred ACHPN certified or willing to obtain.

Responsibilities

  • Maintains the confidentiality of patient and employee information at all times, both at work and off duty, and after the termination of employment. According to the agencies Notice of Privacy Practices and HIPAA regulations.
  • Is knowledgeable and functional in computer applications including database, word processing, spreadsheet, and e-mail system as required for the execution of the job.
  • Facilitate the resolution of problems and conflict by encouraging decision making by those responsible, remaining objectives, respecting feelings, and soliciting perspectives to each new idea.
  • Facilitate an active collaboration model in the provision Palliative Care Service with the Palliative Care IDT and actively involved physician(s).
  • Performs appropriate evaluation of patients who are referred for Palliative Care services, and assist with the diagnosis, treatment and management of pain and symptoms related to serious illness associated with acute and chronic health conditions.
  • Provides services related to palliative care symptom management, and, if necessary, end of life care. This includes appropriate education of patient, caregiver, and family to promote best health possible.
  • Orient, train and provide continued staff education and development to ensure staff awareness of care and services provided by Palliative Care services.
  • Consistently communicates in a clear, accurate, direct, open and timely manner in all aspects of the position, within the agency and with external contacts.
  • Promote teamwork by keeping others informed, participation effectively in group decision making, working to accomplish team objectives and projects, and by soliciting feedback about one’s effectiveness as a team member.
  • Consults with the medical director, physician or designees, inform primary/specialty physician of services provided and collaboration with another physician if ordered: that patient status reports are timely, accurate and consistent.
  • Demonstrates knowledge and utilization of clinical interventions appropriate to the management of pain and other symptoms experiences by palliative care patients.
  • Prepare and maintain accurate patient records, charts and documents to support sound medical practice and reimbursement for services provided.
  • In collaboration with the Palliative Care IDT, participates and contributes in ongoing quality initiatives, the clinical growth of the program, the strategy of program plans or development, and includes enhancements to the existing program and ensures that departmental and team goals/objectives are met.
