About The Position

As a Palliative Care Nurse Practitioner at Allina Health, you are a valued team member serving a significant and special role in our patients' and their loved ones' care... Principle Responsibilities Completes comprehensive patient assessments and determines diagnosis and treatment of illness. Orders and interprets medical diagnostics in tandem with the attending physician. Prescribes medications, treatments, therapy, and medical equipment. Conducts patient education related to chronic illness or wellness care. Consults with a physician when the patient's needs are outside of APRN's scope of practice, or if the diagnosis or treatment plan is unclear. Refers for specialty care when determined. Provides communication and acts as a resource within the team and other interdisciplinary health care staff. Works with the primary care team for program development. Acts as liaison with outside agencies as needed. Collaborates with team members in other specialty areas by consulting and rounding on patients, depending on their needs Monday - Friday, regular business hours Will float to other sites if needed, depending on staffing/patient needs No weekends or on-call

Requirements

  • Master's degree - Completion of an accredited Nurse Practitioner program
  • Certified Nurse Practitioner - American Nurse Credentialing Center (ANCC) certification within specialty required upon hire
  • Certified Nurse Practitioner - MN Board of Nursing required upon hire
  • ACLS-BLS Tier 2 - Multisource required within 180 Days
  • Drug Enforcement Administration required upon hire
  • Valid Driver’s License required upon hire - MN and WI residents must obtain a valid driver’s license in their state of residence within 60 days of employment for those working in Complex Care for Seniors, Community Palliative Care and Hospice

Nice To Haves

  • Minimum 2 years of experience in palliative care, internal medicine, family practice, hospital medicine, or oncology
